Momentum for Health
Dennis Adjei is a skilled Case Manager with extensive experience in mental health support and community rehabilitation. Currently employed at Momentum for Mental Health since June 2019, Dennis manages a caseload of over 40 clients, establishing personalized treatment plans and facilitating therapeutic groups. Previous roles include Service Coordinator/Case Manager at Crestwood Behavioral Health, Inc., where Dennis assessed and created care plans, and Support Specialist at Employment and Community Options, focusing on supportive interventions for individuals with various disabilities. Dennis holds a Bachelor of Science in Nursing from Holy Names University and an Associate's Degree in Psychology from De Anza College, combining a strong educational background with practical experience in client care and behavioral support.

The mission of Momentum for Health is to help individuals achieve mental and emotional health, discover and reach their potential, and fully participate in life. Momentum builds stronger communities by giving individuals affected by mental and emotional illness treatment, support, and opportunity to lead healthier lives. Our work is based on the knowledge that individuals who have a mental illness can, and do, recover through available, effective, culturally competent services that encourage self direction and hope. We are a private, nonprofit organization that relies on the generosity of individuals, foundations and corporations to support our services.
