Lydia Nesemann

Lydia Nesemann has extensive experience in the healthcare industry. Lydia is currently working at Monogram Health as the Senior Vice President of MTM & Pharmacy Performance. In this role, they lead the Comprehensive Medication Management Pharmacy team and focuses on improving drug therapy outcomes for patients with kidney disease. Before joining Monogram Health, Lydia worked at Aetna, a CVS Health Company, where they played a key role in new business development for Aetna Medicaid Pharmacy. Prior to that, they held leadership positions at Health Choice, serving as the Vice President of Pharmacy and the Pharmacy Director at Health Choice Management Company.
