Suzanne Wixom currently serves as the Home Health Care Manager II at Monroe Plan for Medical Care since August 2022, focusing on coordinating care for Medicaid and Dually Eligible Medicaid/Medicare enrollees. In addition, Suzanne has been a member of the NY State Chaplain Task Force since May 2021. Prior experience includes the role of Reintegration Program Manager at the Addiction Care Center of Albany, where responsibilities included substance abuse counseling, as well as serving as a Case Manager at Conifer Park. Earlier positions include Food Service Director at Niskayuna Central School and Master Instructor at New York State Professional Cooking School. Suzanne holds a CASAC Certification in Substance Abuse/Addiction Counseling from ACCA, earned between 2018 and 2019.
