Daniela Bernal is an experienced outreach professional with a strong background in community engagement and health services. Currently serving as an Outreach Specialist at myPlace Health since August 2023, Daniela previously held the position of Community Outreach Coordinator at ConcertoCare from March 2022 to August 2023. Daniela's experience includes roles at L.A. Care Health Plan, where responsibilities ranged from Community Representative II to Member Services Representative and Department Assistant. Additional experience includes working as a Front Office Coordinator at UC Berkeley and a Front Desk Receptionist in the UC Berkeley Psychology Department. Daniela's advocacy work is exemplified by time spent as a Family Advocate at Health Leads. Daniela earned a Bachelor's Degree in American Studies with a focus on Public Health and Global Poverty and Practice from the University of California, Berkeley, completing the program in 2016.

myPlace Health was founded in 2021 by mission-driven healthcare leaders and organizations (including SCAN Group and Commonwealth Care Alliance) that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. myPlace Health is an integrated care delivery organization that specializes in providing personalized care and coverage to older adults who wish to remain safely living in their homes and communities for as long as possible, rather than living in a nursing facility. Our mission is simple: to enable older adults to live the independent lives they deserve. The primary ways in which we pursue our mission are through capitated arrangements with government payers, such as Medicare and Medicaid under the PACE (Program of All-Inclusive Care for the Elderly) model, and as a value-based provider working in partnership with local health plans.
