Theresa Reyes

Theresa Reyes is a skilled Palliative and Hospice Nurse Practitioner currently employed at National Church Residences since September 2022. Prior to this role, Theresa held several positions at Mount Carmel Health System, including Director of Nursing and Clinical Operations for Hospice and Palliative Care, and Practice Manager for Advance Practice Providers, from July 2010 to September 2022.
