Gemma Fetalver

Gemma Fetalver has extensive experience in social work and community liaison roles within healthcare settings. Currently serving as the Community Liaison at Nazareth Home since July 2018, Gemma previously worked as a Transition Liaison at Signature Healthcare, where responsibilities included conducting clinical field evaluations of patient referrals and assessing patient needs. Prior positions include Clinical Liaison at Elmcroft Senior Living, Medical Social Worker at Baptist Health Louisville, and a variety of roles in medical social work and case management. Gemma holds a Master of Science in Social Work and a Bachelor of Arts in Sociology from the University of Louisville.
