Neighborhood Family Practice

Neighborhood Family Practice was founded in 1980 in response to a concern that residents of Cleveland’s near west side lacked access to primary health care. A Federally Qualified Health Center (FQHC), NFP focuses on providing high quality primary care in a medical home setting. As a health care provider, our seven convenient locations offer same day appointments. The NFP service area spans twelve neighborhoods across Cleveland and Lakewood and is comprised of a very diverse mix of residents. NFP accepts Medicaid, Medicare, most commercial insurance plans and offers a financial assistance program (sliding fee scale) for the uninsured. Neighborhood Family Practice is a grantee of the Health Resources and Services Administration (HRSA) and each of its locations are covered by the Federal Tort Claims Act (FTCA).
