New Choices assists persons with disabilities to live and participate in their communities. Our services are available on an as needed basis, up to 24 hours a day for person of all ages who qualify. New Choices Vision- Communities will acknowledge and respect the right of people with disabilities and will provide the resources and opportunities necessary for them to achieve their life goals. New Choices, Inc provides services to individuals with a wide variety of people including those with: * Intellectual Disabilities * Brain Injury * Mental Illness * Physical Disabilities * Developmental Disabilities * and the Elderly. The services we offer are Supported Community Living, Respite, Skilled Nursing, Home Health Aide, IMMT, CDAC and Home based Habilitation
