Nicole Jordan-Martin

Nicole Jordan-Martin, MPA, FACHE, currently serves as the Executive Director and Chief Executive Officer of Community Care at NYC Health + Hospitals, a role held since June 2015, where responsibilities include overseeing the operational and financial performance of the Medicaid Health Home program. Prior positions include Deputy Executive Director roles in both Community Care and At Home, focusing on care management and coordination improvements, and leading a workforce of over 200 members. Additionally, Nicole has substantial experience in the DSRIP Program initiatives as the Senior Assistant Vice President at NYC Health + Hospitals and held the position of Senior Director and Executive Director at Montefiore Health System, facilitating Medicaid care coordination initiatives. Educational qualifications include a Master of Public Administration with distinction in Health Administration from Long Island University, a Bachelor of Science in Health Administration from St. Joseph's College, and a practical nursing diploma from Clara Barton Vocational High School.
