Sebastian Flak

Sebastian Flak is a Clinical Pharmacist at OHI - Ocean Health Initiatives, Inc. since February 2019, bringing over nine years of experience in pharmacy roles including Per Diem Pharmacist, Pharmacist in Charge, and Pharmacy Intern at CVS Health from May 2010 to October 2019. Flak has also completed externships at various reputable organizations, including Acute Care Pharmacy Extern at Capital Health System, Hospital Pharmacy Extern at Robert Wood Johnson University Hospital, and Long Term Care Pharmacy Extern at PharmCare USA. Educational credentials include a Doctor of Pharmacy (Pharm.D.) from Rutgers University, completed in 2014. During the course of the career, Flak has demonstrated expertise in prescription processing, patient counseling, medication management, and conducting community outreach initiatives.
