onehome allows patients to recover where they feel most comfortable: in their homes. As a leader in value-based care, onehome caters to both providers and patients, through delivering customized home healthcare, durable medical equipment, and infusion services. Our Approach: Empathetic healers and industry change makers, we make recovering in the home revolutionary for patients – who experience better health outcomes and reduced hospital re-admissions, and for providers, who experience better care coordination and care management. As a single point of accountability for all home healthcare needs, onehome oversees the entire scope of patient care, from referral to recovery. Our Promise: Cost effective, reduced administrative burden, reduced equipment waste, and better patient outcomes – with improving quality of life at our forefront. We know that the road to recovery is complex, which is why we chose to bring healing home. Now, patients and providers no longer need to worry about fragmented healthcare, lack of transparency, and long hospital stays. Our entire staff – from our nurses to therapists and medical equipment specialists are available 24/7 to ensure patients recover with their loved ones, and remain independent in their homes. Our Success: We served over 2 million members in four states – and we are growing. After a successful seven year partnership, we were acquired by Humana to serve more members and bring our value based care model to more doorsteps.
