Through disease management, we prevent unnecessary or repetitive medical care in conditions such as Depression, Dementia, Chronic Obstructive Pulmonary Disease (COPD) and Diabetes. Caregiver support and fall management programs are also available. We strive to ensure that the client receives all the necessary services to promote health and maximize well-being while helping the client remain independent.
