Our Hospice is hiring a Weekend Option Supervisor (RN) for our Inpatient Facility (IPF). This person will triage the after hours team while allowing flexible on-site hours while supervising our Inpatient Facility (IPF). This role will be responsible for working closely with the Patient Care Manager (PCM), Nurse Practitioners, and clinical staff for quality of care of Hospice patients and families.

Shift:

Supervise our After Hours team starting Friday, 4:30pm to Monday, 8:30am. Flexible in-person hours in the IPF, on Saturday and Sunday are required.

A Weekend Option Supervisor (RN) at Our Hospice will:

  • Triage incoming phone calls from patients, families, facilities, etc. to ensure hospice services are provided according to regulations and standards established by OHSCI.
  • Coordinate and provide the delivery of services after-hours in order to provide a cost-effective, quality healthcare delivery system during weekend hours.
  • Provides Discharge Planning for patients leaving the IPF and returning to their place of residence.
  • Provides problem solving and decision-making to the RN, HHA, and other After Hours weekend staff with direction from PCM and Director of Clinical Operations.
  • Communicates with and updates other IDG team members as needed to coordinate and ensure quality care social services, home health aides, volunteers and medical staff.
  • Responds timely to all referrals, fostering relationships of trust, responsiveness, and open communication with all referring customers including but not limited to hospital case managers, nursing facility administrators and staff, physicians and physician office staff, patients and families.

A Weekend Option Supervisor (RN) at Our Hospice Needs:

  • Associate or Bachelor Degree in Nursing (ASN/BSN)
  • Current Registered Nurse License in Indiana
  • One to three years’ experience in health care preferred

Job Type: Full-time

Pay: From $38.00 per hour
