Onboarding Manager, Case Management at Paradigm

Rachel Fortin has a diverse work experience spanning over a decade. Rachel started their career at Mayo Clinic in 2010 as a Registered Nurse in the Surgery department, where they worked until 2014. During this time, they gained experience in patient care coordination, education, data management, quality assessment, and program development. From 2014 to 2019, Rachel served as an RN Liver Transplant Coordinator at Mayo Clinic, responsible for coordinating care for transplant patients and their families. In 2019, they briefly worked as an RN Care Manager at A Hand To Hold - Home Care. Rachel's most recent role is at Paradigm, where they currently hold the position of Onboarding Manager, Case Management, since November 2022. Prior to this, they were a Case Management Nurse at Paradigm from October 2020 until December 2022.

Rachel Fortin has a comprehensive education history in the field of nursing. Rachel obtained their Associate's Degree in Registered Nursing/Registered Nurse from Parkland College between 2007 and 2009. Following this, they pursued a Bachelor's Degree in Nursing from Illinois State University-Mennonite College of Nursing, which they completed between 2011 and 2012. Rachel then furthered their education by obtaining a Master's Degree in MSN Nursing Education from Jacksonville University from 2014 to 2015.

In addition to their degrees, Rachel has also obtained several certifications. Rachel became a Registered Nurse certified by the Florida Board of Nursing in March 2010. Furthermore, they obtained the Certified Clinical Transplant Coordinator certification from the American Board for Transplant Certification in July 2015. While a specific month and year are not provided, Rachel also holds a certification as a Certified Case Manager from the Commission for Case Manager Certification (CCMC).
