Pathways to Housing PA empowers people with disabilities to improve their housing stability, achieve better health, and reclaim their lives.

We start by housing people directly from the streets, without precondition.  Then we address their underlying issues around mental health, addiction, medical care, and income to help integrate and welcome them back into our community.

The Support Services team at Pathways to Housing PA provides comprehensive, client-centered care to individuals transitioning from homelessness to stable housing. This multidisciplinary team, comprising specialists in community inclusion, mental health, nursing, and program coordination, collaboratively addresses clients' diverse needs, focusing on mental health, addiction recovery, and medical care. They work to empower clients to enhance their well-being, secure stable income, and successfully reintegrate into the community.
