Heather Byrnes

Heather Byrnes has a diverse work experience in the healthcare and social services sector. Heather started their career as a Summer Support Worker at Autism Services, Inc. in 2003. Later, they worked as a Support Worker at Childcare Resources from 2003 to 2005. Heather then joined Community Living St. Catharines as a Support Coordinator from 2005 to 2007. After that, they worked at Bethesda Services as both a Support Worker and an Instructor Therapist from 2006 to 2008. From 2008 to 2011, they served as an Autism Therapist at their own company, Heather Byrnes. Heather then transitioned to NYSARC, Inc. dba The Arc Erie County New York, where they held various roles, including Medicaid Service Coordinator, Senior Medicaid Service Coordinator, and Medicaid Service Coordinator Supervisor from 2011 to 2018. Currently, they are working at Person Centered Services Care Coordination Organization, LLC as the Director of Quality & Compliance. Prior to this role, they held positions as the Regulatory Compliance Coordinator and Care Coordinator Supervisor at the same company.

Heather Byrnes completed their high school education at Chelmsford Valley District Composite School from 1995 to 2000, earning a High School Diploma. Following that, they attended Laurentian University/Université Laurentienne from 2000 to 2004, where they obtained a Bachelor of Arts degree in Psychology.

Person Centered Services Care Coordination Organization, LLC

Person Centered Services provides tailored care coordination services to individuals with Intellectual and/or Developmental Disabilities (I/DD). When New York State recognized the need for an improved, comprehensive, and conflict-free care management model for individuals with intellectual and developmental disabilities, Person Centered Services answered the call. We are a Care Coordination Organization (CCO) that was formed by 31 existing developmental disability service providers and agencies, and today we employ over 600 highly experienced care coordinators with offices in each of the 18 counties we serve. Care Coordination connects the dots between all aspects of an individual’s care. A Life Plan, a holistic plan tailored to the individual is created to encompass every aspect of an individual’s care, including disability-related supports and other services like medical, dental, wellness, behavioral health, job assistance, housing, safety, transportation and more. Plans are constantly monitored and modified as an individual's goals, needs and life circumstances change. At Person Centered Services we're all about serving people. Our mission is to connect individuals to the care, support and opportunities that maximize their quality of life so they can reach their potential.
