Person Centered Services provides tailored care coordination services to individuals with Intellectual and/or Developmental Disabilities (I/DD). When New York State recognized the need for an improved, comprehensive, and conflict-free care management model for individuals with intellectual and developmental disabilities, Person Centered Services answered the call. We are a Care Coordination Organization (CCO) that was formed by 31 existing developmental disability service providers and agencies, and today we employ over 600 highly experienced care coordinators with offices in each of the 18 counties we serve. Care Coordination connects the dots between all aspects of an individual’s care. A Life Plan, a holistic plan tailored to the individual is created to encompass every aspect of an individual’s care, including disability-related supports and other services like medical, dental, wellness, behavioral health, job assistance, housing, safety, transportation and more.
