Jordon Moore, PT, DPT, OCS, FAAOMPT has served as a Facility Director at PHOENIX Rehabilitation and Health Services, Inc. since June 2019. Additionally, with previous experience as a Center Manager/Physical Therapist at Select Physical Therapy.
Jordon Moore received their Doctor of Physical Therapy degree from Shenandoah University in 2008. In 2013, they obtained the certification of Fellow of The American Academy of Orthopaedic Manual Physical Therapists from IFOMPT.

Phoenix was founded in 1997 to set a new standard for outpatient physical therapy.
