Founded in 2012, Physician Housecalls is an organization designed to meet the unique needs of home-bound patients. Our purpose is to care for people when they're the most vulnerable in the place they're the most comfortable, their home. Thus, providing everyone access to the support they need for a healthier and more fulfilling life. We believe home based primary care is the missing piece of the healthcare puzzle. In the home, clinicians are able to assess the patient’s social determinants of health, which allows them to create a more effective treatment plan to fit the patient’s needs. And with recent changes in reimbursement and advancing technology, it is now feasible to bring our care to the patient rather than requiring them to travel to us. Currently, our focus is caring for our ever growing geriatric population. A large majority of this population face a serious care issue as they are unable to see a doctor due to transportation and mobility limitations. These limitations put them at a high risk of routine ER visits and hospitalization,as their multiple chronic illnesses are not monitored frequently enough to keep them stabilized. Our model of care solves the limitation issue by bringing the care to the patient. To take it a step a further, we also partner with home health agencies, hospice agencies, mobile lab & x-ray services, and many other entities to ensure our patients receive the support they need to remain safely at home.
