Our services reach all ages from early childhood to geriatric care and for the most part is hugely responsible for helping clients remain in their home and avoid placement in a higher level of care, such as residential placement or hospitalization. Without case management services, many, if not most, of the clients being seen would not be able to maintain the "normalcy" of daily living.
