Prime Care Coordination
Melissa Marletta has a Master of Science Degree in Mental Health Counseling/Counselor from St. John Fisher University. Melissa worked as a Primary Therapist at Rochester Regional Health, where Melissa provided mental health counseling to teenagers and adults. Prior to that, Melissa was a Community Habilitation Provider at Catholic Charities Community Services, working with school-age clients with developmental disabilities. Currently, Melissa is a Care Manager at Prime Care Coordination, where Melissa works with the ID/DD population of all ages, providing services remotely and in person.
Prime Care Coordination
Prime Care Coordination began as a means to prepare for the shift to care coordination and managed care for people with intellectual and developmental disabilities. Through the collaboration of providers in local communities. Prime Care Coordination was launched to assist people with intellectual and developmental disabilities with managed care and care coordination.
