Prime Care Coordination
Nicole Simmons is currently working as a Direct Support Professional at Heritage Christian Services. Nicole also serves as a Care Manager Assistant at Prime Care Coordination. Nicole has experience in teaching independent living skills to support individuals in maintaining clean and safe homes, as well as promoting community involvement. Additionally, Nicole has past internship experience at Monroe Community College, where Nicole assisted with fieldwork assignments and administered exams. Nicole also worked as an Assembly Line Worker at DELPHI, where Nicole met daily quotas and managed machinery. Nicole holds an Associate's Degree in Human Services from Monroe Community College.

Prime Care Coordination began as a means to prepare for the shift to care coordination and managed care for people with intellectual and developmental disabilities. Through the collaboration of providers in local communities. Prime Care Coordination was launched to assist people with intellectual and developmental disabilities with managed care and care coordination.
