The Healthcare Operations team at PrimeCareHealth Community Health Centers is responsible for overseeing and optimizing the delivery of medical and behavioral health services across five accessible locations. This multidisciplinary team ensures seamless coordination between primary, specialty, and acute care settings, with a strong emphasis on quality and practice transformation. They manage family medicine care models, prenatal services, complex chronic disease management, and integrated behavioral health, while also focusing on culturally competent care for underserved populations, particularly Latino and Polish communities. The team drives initiatives in outreach, enrollment, and patient population health management to enhance overall care delivery and patient outcomes.
