Leslie Nguyen

Leslie Nguyen is a dedicated healthcare professional with extensive experience in medical management and care coordination. Since August 2020, Leslie has served as a Medical Management Personal Care Coordinator at Santa Clara Family Health Plan, focusing on patient outreach, health risk assessments, and collaboration with case managers. Additionally, since June 2016, Leslie has worked as a Care Provider at Premier Health Services, Inc., assisting clients with autism in daily routines and life skills development.
