Nicole Dominguez

Nicole Dominguez, CCM, LCSW, is an accomplished professional in the field of social work and behavioral health, currently serving as the Manager of Behavioral Health at Santa Clara Family Health Plan since July 2020. In this role, Nicole oversees a team of clinical case managers and personal care coordinators while fostering partnerships with community organizations. Additional experience includes positions such as Manager of Behavioral Health (Interim) and Psychotherapist in Private Practice at Grow Therapy, as well as roles at Teladoc Health, BetterHelp.com, and Good Samaritan Hospital as a Licensed Clinical Social Worker. Prior experience includes providing psychosocial support as a Medical Social Worker at Heartland Hospice Services and a variety of case management roles. Nicole holds both a Master's degree and a Bachelor's degree in Social Work and Behavioral Sciences from San Jose State University.
