Skyland Trail is a nonprofit mental health treatment organization based in Atlanta. For more than 30 years, we have been inspiring people with mental illness to thrive through a holistic program of evidence-based psychiatric treatment, integrated medical care, research and education.

Skyland Trail serves adults ages 18 and older and adolescents ages 14 to 17.

We’re not a hospital or lock-down institution, we’re one of the nation's top mental health treatment centers – a voluntary treatment community of clients finding new ways to overcome the limitations of their illness and reclaim their lives.

But success is built on a two-way commitment – we’re committed to our patients and we ask our patients to commit to our approach. Each client has to put in the time and effort needed to make real and sustainable progress.

The Clinical and Care Coordination Team at Skyland Trail is responsible for the comprehensive management and coordination of patient care. This includes assessing, planning, and implementing individualized treatment plans, integrating medical and psychiatric care, and facilitating therapeutic activities. The team ensures a seamless, holistic treatment experience by collaborating across disciplines and maintaining continuous communication with clients and their families to support ongoing recovery and well-being.
