Nicole de la Fuente

Nicole de la Fuente, LCSW, NSW-C, is a seasoned social worker with a focus on pediatric nephrology and transplantation. Currently serving as a Senior Case Manager at Son of a Saint since June 2023, Nicole previously held the position of Pediatric Nephrology Social Worker at Children's Hospital New Orleans from March 2021 to July 2023, where responsibilities included conducting psychosocial assessments and enhancing patient support in dialysis. Nicole's experience also encompasses roles as a Kidney Transplant Social Worker at Ochsner Health, and as a social work intern at Ochsner Health System, where patient assessments and resource referrals were key duties. Earlier, Nicole contributed to community awareness about blood cancers through impactful campaigns at The Leukemia & Lymphoma Society. Nicole earned a Master of Social Work (MSW) from Tulane University School of Social Work and holds a degree from Louisiana State University.
