Ashley Zapp

Ashley Zapp, LCSW, currently serves as the Manager of Acute and Post-Acute Care Coordination at St. Peter's Health Partners since April 2023. Alongside this role, Ashley has been the Manager of Care Coordination & CIN APM at INNOVATIVE HEALTH ALLIANCE OF NEW YORK, LLC since February 2021 and has held a position as a Field Instructor and Research Assistant at the University at Albany, SUNY, since September 2013, focusing on research related to perception and cognition in the deaf with schizophrenia. Previous positions include Social Work Supervisor for Acute Care at St. Peter's Health Partners and Clinical Social Worker, as well as roles at Columbia Memorial Health and Coarc, where Ashley provided intensive support and care coordination. Ashley earned a Bachelor's Degree in Psychology from Syracuse University and a Master's Degree in Social Welfare from the University at Albany.
