Candace Chin

Candace Chin, PharmD, BCPS, currently serves as a Decentralized Pharmacist and Per Diem Pharmacist at St. Peter's Health Partners and HealthAlliance of the Hudson Valley, roles held since October 2017. Prior experience includes working as a PGY-1 Pharmacy Practice Resident at Elliot Hospital from June 2016 to June 2017 and as a Pharmacy Intern at Aureus Health Services from March 2014 to December 2015. Candace Chin earned a Doctor of Pharmacy (Pharm.D.) from the University at Buffalo Pharmacy and Pharmaceutical Sciences in 2016, and completed undergraduate studies at Binghamton University from 2009 to 2012.
