David Baltazar

David Baltazar is an experienced Family Nurse Practitioner with a comprehensive background in healthcare spanning over 20 years. Currently serving in dual roles at The Center for Health Care Services and UnitedHealth Group since November 2016 and August 2017 respectively, David has also held positions as a Nurse Practitioner at Ageless Men's Health and as an RN in various emergency and hospital settings, including First Choice Emergency Room and Plantation General Hospital. Educational qualifications include a Master’s Degree in the Family Nurse Practitioner Program and a Bachelor’s Degree in Registered Nursing from Texas Tech University Health Sciences Center, along with an Associate of Science in Registered Nursing from Broward College.
