ThoughtSwift Medical Assessments (ThoughtSwift) is an early stage medical software provider of Medical Assessments to Primary Care & Internal Medicine Physicians,.The T.S. program gathers key psychological information provided by the patient as to their current mental and psychological conditions from a series of questions on an IPAD orcomputer. It instantly assesses (does not diagnose) the patient's well being and overall state of mind. A second program , the Annual Wellness Report gathers information and produces a report physicians should be using to build a five year Chronic Care plan for Medicare patient. A third program the Chronic Care Tracker like all of the companies programs are mandated as Standard of Care for all physicians seeing patients. Every Senior patient is entitled to these reports once per year, and every patient under 65 with commercial health insurance are entitled an annual psychological assessment once per year . The average physician sees 3000 patients per year, the range of reimbursement to the physician is $64.00 to $240.00, earning $150-450,000 of new revenue from existing patients, without overburdening his or staff support time, and without major capital investment, not to mention it established medical necessity for multiple additional follow up physician-patient visits.. Beyond the program's direct economic and medical benefits the T.S. program helps assure physician compliance of today's need to demonstrate on going efforts to provide Preventative & Wellness benefits to their patients which result in maintaining their Heddia scores, required by the insurance companies and enforced by ACA.
