Post Acute Care Rehabilitation (PACR) Program provides quality and integrated care for patients: who are recovering from orthopedic surgery, hip and other fractures; those with medically complex conditions and post-stroke complications affecting motor skills; and those who have suffered stroke and neurological issues. Our dedicated team of skilled inter-professional health team includes Physicians, Rehab Nurses, PT, OT, SLP, Communication Disorders’ Assistant, Recreation Therapist, Social Worker, Chaplain and Clinical Dietitian. A central therapy room is available to support an integrated therapy plan and rehabilitation. Persons accepted into PACR look forward to returning home and/or back into the community.
