Dori Isakow-Weiss has extensive work experience as an Occupational Therapist with a focus on patient care and management. Dori began their career as a Clinical Occupational Therapist at Toronto Grace Health Centre in November 2007, where they performed assessments and implemented treatment programs for patients with various injuries, disorders, and diseases. Dori also provided consultation services on the Palliative Unit. In January 2019, Dori took on the role of Patient Care Manager and Allied Health Manager at the same company. As an Occupational Therapist, they worked at HealthMax Physiotherapy Clinics starting in December 2013, specializing in in-home attendant care assessments and treatment plans for clients involved in motor vehicle accidents and slip and falls. Additionally, they served as an independent assessor for clients who had sustained whiplash injuries in a motor vehicle accident at Herrold & Vernon Evalworks from February 2008 to January 2011, conducting in-home assessments, providing ergonomic education sessions, completing Form 1s, and facilitating hospital discharges to clients' homes.

Dori Isakow-Weiss earned a Bachelor of Arts (BA) degree in Kinesiology from York University, where they studied from 2002 to 2005. Following their undergraduate studies, they pursued a Master's degree in Occupational Therapy/Therapist at the University of Toronto, which they completed from 2005 to 2007.

Toronto Grace Health Centre (TGHC) is a 150-bed facility owned and operated by The Salvation Army located in the heart of downtown Toronto. Through understanding and appreciating the complexity of day-to-day life, Toronto Grace provides patient-centered care for individuals with multifaceted chronic diseases who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care Services. Our Complex Continuing Care (CCC) program serves persons with complex and multiple chronic diseases and functional disabilities who require a wide range of therapeutic services and clinical specialties. The CCC program provides medical management, skilled registered nursing care, as well as therapy, recreation and spiritual services. The program offers care of the individual following a restorative and preventative approach. Post Acute Care Rehabilitation (PACR) Program provides quality and integrated care for patients: who are recovering from orthopedic surgery, hip and other fractures; those with medically complex conditions and post-stroke complications affecting motor skills; and those who have suffered stroke and neurological issues. Our dedicated team of skilled inter-professional health team includes Physicians, Rehab Nurses, PT, OT, SLP, Communication Disorders’ Assistant, Recreation Therapist, Social Worker, Chaplain and Clinical Dietitian. A central therapy room is available to support an integrated therapy plan and rehabilitation. Persons accepted into PACR look forward to returning home and/or back into the community. Palliative Care provides comprehensive end-of-life care to support the person and family at their end of life journey. Our holistic approach to care includes a skilled interdisciplinary team to meet the individual’s physical, psychosocial, spiritual and cultural needs. Persons in the community who wish to remain at home for as long as possible can be rapidly admitted to the Palliative Care Unit if needed.
