Jackie Caldwell

Jackie Caldwell is an experienced healthcare professional with a background in care management and transitional care. Currently serving as a Care Manager and Transitional Care Specialist at Trinity Health since May 2018, Jackie also holds the position of Care Navigator at HealthPlus of Michigan since September 2013, where responsibilities include coordinating education and follow-up care for members. Previous roles include Care Navigator at Davis-Smith, LPN Supervisor at BriarwoodCare Center, and MDS Nurse/Scheduler at Saginaw Geriatrics, with a focus on client assessments and quality of care. Jackie’s early career involved educating high school students in healthcare vocational programs at Flint Community Schools.
