Lisa Simmons-Fields

Lisa Simmons-Fields, DNP, currently serves as President of CMSA Detroit, a position held since January 2024. Concurrently, Lisa is the Director of System Population Health/Care Management at Trinity Health, a role taken on in March 2018. Prior to these positions, Lisa was President-Elect of the Case Management Society of America-Detroit Chapter from January 2022 to January 2024. Lisa's experience includes key leadership roles at St. John Providence, where responsibilities included Director of Quality for the Ambulatory Network and Director of Care Management, focusing on the development of integrated clinical care management models. Lisa began management responsibilities in care management at St. John Hospital & Medical Center in January 2011. Lisa holds a Doctoral Nurse Practitioner (DNP) degree in Population Health Management from Madonna University and a Master of Science in Administration (MSA) from Central Michigan University.
