Eric Boehm

Eric Boehm has extensive experience in healthcare coordination and patient relations, currently serving as a Provider Relations Specialist at Unify Health Services LLC since August 2023. Prior to this role, Boehm worked at CORA Health Services from September 2016 to July 2023 as a Care Coordinator, where responsibilities included staffing patient referrals and conducting patient surveys for a Medicare Bundle program. Previous experience includes coordinating post-acute care services at AdventHealth from January 2012 to August 2016, as well as assisting case managers during patient transfers. Boehm has also completed internships focused on patient access and case management. Academic credentials include a Master’s Degree and a Bachelor of Science in Health Services Administration, both obtained from the University of Central Florida.
