Jacqueline Crouch

Jacqueline Crouch, PharmD, BCPS, BCPPS, serves as the Clinical Pharmacy Manager and Lead Pediatric Intensive Care Pharmacist at Valley Children's Healthcare since July 2012. Starting as a PGY-2 Critical Care Pharmacy Fellow, Crouch transitioned to a Neonatal and Pediatric Critical Care Pharmacist and has held the Lead PICU Pharmacist position since May 2016. Responsibilities encompass CPOE order verification, dosing and monitoring for various critical care medications, attending ICU rounds, and medication reconciliation, along with precepting pharmacy interns and residents. Crouch also works per diem at Bullard Pharmacy, holds an Assistant Clinical Professor position at the University of California, San Francisco, and serves as an Adjunct Professor at the University of the Pacific. Educational qualifications include a Doctor of Pharmacy from the University of the Pacific and a Bachelor of Science in Biology from California State University, Fresno.
