Jennifer Cicconetti, PharmD, BCPS, BCGP, BCMTMS, currently serves as the Chief of Pharmacy and Director of Pharmacy at Valley Health Partners Community Health Center since April 2022. Prior to this role, Jennifer held positions as Manager of Care Coordination - Pharmacy and Clinical Pharmacist at Populytics from November 2019 to April 2022. Experience earlier includes serving as a Clinical Pharmacist in Population Health and Clinical Staff Pharmacist at Lehigh Valley Health Network from July 2007 to November 2019, and various pharmacy management roles at Rite Aid, Hartzell's Pharmacy, and Arrow Pharmacy and Nutrition Centers throughout the late 1990s and early 2000s. Educational qualifications include a Doctor of Pharmacy from the University of Florida (2010-2013) and a Bachelor's degree in Pharmacy from the University of the Sciences in Philadelphia (1993-1998).

Valley Health Partners Community Health Center (VHPCHC) is a nonprofit organization recognized as a Federally Qualified Health Center Look-Alike (FQHC-LA) by Health Resources & Services Administration (HRSA). An FQHC-LA is a community health center that provides comprehensive primary care and other services such as interpretation, transportation and health education for the community population. An FQHC-LA does not turn patients away based on their ability to pay and removes financial barriers for receiving care. FQHC-LAs provide whole-person care, creating opportunities for the health center to look beyond medical care and focus on social determinants of health that could be some of the underlying causes of poor health. VHPCHC is proud to offer these services to all members of our community!
