Heather Layfield, LPN, currently serves as the Outreach Manager at Valor Lakes since November 2024, following a role as Transition Liaison at Orlando Health from August 2022 to October 2024. Previously, Layfield worked as a Transition Liaison for both SkyTop View Rehabilitation Center and South Lake Home Health from May 2015 to January 2019, facilitating clinical reviews to ensure appropriate patient transitions post-hospital discharge. To free veterans from America’s mental health and addiction crisis and empower them to create a better future for themselves and their families Valor Lakes’ intensive outpatient program (IOP) offers flexible mental health and addiction treatment options for veterans and their immediate families. Valor Lake’s IOP includes individual and group therapy sessions, addiction education, relapse prevention planning, and support groups for veterans and their family members. IOP is an effective option for individuals who prefer non residential treatment or who have completed an inpatient program and require additional support. Our Extended Care Program helps our clients continue their recovery journey in a safe and structured environment. This program preserves veterans’ independence as they build a better future. The focus of this program includes: • Trauma-focused psychotherapy • Psychoeducation: CBT/DBT/ACT • Group Therapy • Relapse Prevention • Case Management • Medication Management • Life Skills Development • Family Therapy • Social Skills Development
