Sarah Collins

Sarah Collins, MSW, LSW, CMC, is an experienced hospice social worker currently employed at VITAS Healthcare since July 2017, providing psychosocial support within an interdisciplinary team. Previous roles include serving as a Certified Aging Life Care Manager at ElderTree Care Management, where comprehensive in-home assessments were conducted, and working as a Certified Compassionate Touch Practitioner to enhance the quality of life for frail elders. Sarah's background also includes coordination of the Primary Care Fellowship Program at National Medical Fellowships, social work services at Hospice of the East Bay, and internships at Masonic Home for Adults and Summit Medical Center. Sarah holds a Master of Social Work with a focus on Gerontology and a Bachelor of Arts in Development Studies, both from the University of California, Berkeley.
