The University of Vermont Health Network - Home Health & Hospice, formerly the Visiting Nurse Association of Chittenden and Grand Isle Counties, is a nonprofit home health and hospice agency serving Vermonters for more than 110 years. We care for 5,000 individuals and families each year. Our programs span lifetimes -- from prenatal care to care for young families to rehabilitation, long-term care, adult day services to end of life. We provide care one person, one family at a time.
