The University of Vermont Health Network - Home Health & Hospice, formerly the Visiting Nurse Association of Chittenden and Grand Isle Counties, is a nonprofit home health and hospice agency serving Vermonters for more than 110 years. We care for 5,000 individuals and families each year. Our programs span lifetimes -- from prenatal care to care for young families to rehabilitation, long-term care, adult day services to end of life. We provide care one person, one family at a time. 

With support from our neighbors, UVM Health Network - Home Health & Hospice provides medically-necessary home and community-based care to individuals and families regardless of their ability to pay. Each year, we provide more than $1 million in charitable care to our neighbors in need.

The Clinical Care Team at The University of Vermont Health Network - Home Health & Hospice provides comprehensive, patient-centered medical care to individuals and families across all stages of life. This multidisciplinary team includes experts in pediatric speech language pathology, home care provision, adult home health, hospice care, and nursing. They coordinate and deliver personalized care plans to address each patient's unique needs, whether for rehabilitation, long-term support, or end-of-life care, ensuring compassionate, high-quality service regardless of patients' financial situations.
