Jessica Reims RN, BSN, currently serves as the Director of Clinical Services Special Programs and also holds the title of Manager of Home Visit Services at Vytalize Health since July 2023. Prior to this role, Jessica worked as a Complex Case Manager at Optum from November 2022 to July 2023. Jessica's experience includes serving as Clinical Integration Nurse Manager at Integrated Care Solutions from April 2020 to November 2022 and various roles at Hartford HealthCare at Home, including Clinical Education Specialist, Nurse Care Coordinator, and Registered Nurse Case Manager from September 2013 to April 2020. Jessica received education from Central Connecticut State University.
