Laurie Baker, BSN, RN, CCM

Laurie Baker, BSN, RN, CCM has worked in the healthcare industry since 2009. Laurie began their career as a Director of Nursing at Sun Healthcare Group, and then moved on to become a Director of Nurses at HCR ManorCare in 2011. In 2013, they were appointed Director of Nursing Services at Kingston Healthcare. In 2017, they took on the role of Director of Nursing at Catholic Health Initiatives. Two years later, they became a Clinical Care Navigator, ACO at The Toledo Clinic. In 2020, they began working as a Transitional Care RN at ConcertoHealth. Most recently, in 2021, they were hired as a Case Management Nurse at Medical Mutual and is currently working as a Chronic Care Nurse at Welby Health.

Laurie Baker, BSN, RN, CCM, obtained their Bachelor's degree in Nursing Administration from Bowling Green State University between 2004 and 2006. In May 2021, they obtained their Certified Case Manager certification from the Commission for Case Manager Certification (CCMC).
