Care Management Team

The Care Management Team at Western Reserve Area Agency on Aging oversees comprehensive care coordination and management services for older adults across Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. The team, consisting of case managers, care coordinators, and transitional care coaches, works to help clients seamlessly transition between care settings, manage their health and wellness, and access long-term services and supports (LTSS) that enable them to age independently in their homes and communities.
