Hannah Rose, LCSW, currently serves as a MSW Guided Supportive/Palliative Care provider at Willamette Vital Health, a role held since June 2023. Prior experience includes positions as a Therapist with Marion County Oregon from October 2020 to June 2023, and as a Medical Social Worker at Samaritan Health Services from December 2019 to September 2020. Hannah also held the role of Case Manager at The Right Door for Hope, Recovery & Wellness between January 2017 and November 2019, and worked at Hope Network from July 2012 to January 2017 in various capacities, including Case Aide and Lead Rehabilitation Technician. Earlier experience as a Direct Support Professional involved promoting independent living for individuals with developmental disabilities. Education credentials include a Master of Social Work (M.S.W.) from Grand Valley State University (2015) and a degree in Interdisciplinary Studies of the Social Sciences from Michigan State University (2012).
