Connie Lynn Yoerges

Connie Lynn Yoerges has a comprehensive work experience spanning several decades. Connie Lynn began their career in 1988 as a member of the United States Army Reserves. Following this, they worked as a Geriatric Nurse at Integrated Health Services from 1994 to 1998, where they provided care to acute and residential patients and served as Interim Director of Nursing. Connie Lynn then joined Life Care Centers of America as an MDS Coordinator from 1998 to 2000. Subsequently, Connie worked at Orange Park Medical Center, where they held two roles: Wound Care Nurse from an unspecified date to 2010, and Telemetry Nurse from an unspecified date to 2005. During this time, they provided direct client care, obtained Wound Care Certification, and participated in various healthcare-related projects and initiatives. From 2010 to 2012, Connie served as a Case Manager at Orange Park Medical Center, conducting client interviews, clinical assessments, and ensuring the alignment of post-discharge services. Connie Lynn then transitioned to SCIO Health Analytics®, where they held the position of Senior Manager, Analytics, from 2012 to 2021. Most recently, Connie joined Brooks Rehabilitation in 2021 as the Manager of Case Management. In this role, they managed the case management team, handled recruitment and training, and provided support to the hospital and staff. Currently, they hold the position of Director at ZignaAI, starting in 2022.

Connie Lynn Yoerges has a diverse educational background. Connie Lynn obtained their Bachelor of Science (BSN) degree in Nursing from Florida State College at Jacksonville. Connie Lynn then pursued further education at the University of Central Florida, where they completed a Master's degree in Social Work with a focus on Social Work.
