Chatrian R. Kanger, MPH is the Senior Vice President of Population Health where she is responsible for aligning resources and clinical information systems to leverage performance improvement initiatives across the network. Chatrian works across partners, health plans, departments and care teams to devise care coordination systems aimed at improving quality, reducing costs, and monitoring patient outcomes. Also in this role, she oversees key programs related to value-based contracting, chronic disease management, data exchange / interface design, and remote patient monitoring. Chatrian brings 20+ years of experience working with health centers, schools, and stakeholders on performance reporting and monitoring, quality improvement, and research design. She was previously a certified Patient Centered Medical Home content expert through the National Committee for Quality Assurance and holds extensive knowledge of clinical quality and outcomes measures. Chatrian has developed content for the National Association of Community Health Centers (NACHC), the Office of the National Coordinator for Health Information Technology, Academy Health, the Journal of the American Medical Informatics Association, Population Health Management, and HIMSS National Health IT Week.
