Tom Hoang

As a Care Manager, Tom specializes in case management, conducting monthly visits with clients and providing assistance with navigating available social services. He came to AFCNS with previous experience at CDF, CCD and residential day programs. Always up for an adventure when he’s not working with his clients, he can be found riding motorcycles, boating, camping and traveling.
