Rafael Yaakobov

Rafael Yaakobov is an experienced Clinical Nurse Case Manager at Mount Sinai St. Luke's and Mount Sinai West since January 2017, also working as a Registered Nurse with AgeWell New York, friendly home care, and sincere care since 2014. Previously, Rafael held a position as a Registered Nurse in the emergency room at North Shore-LIJ Health System from January 2015 to July 2015.
